How It Works
- Step 1 — Begin the Evaluation: Complete payment and provide an email address where your evaluation report can be delivered.
- Step 2 — Complete Intake Forms: Complete structured intake questionnaires and screening measures related to behavioral health, eating patterns, and surgical readiness. The intake may be completed in multiple sittings if needed.
- Step 3 — Clinical Interview and Review: Complete a brief 20-minute structured interview. A licensed clinician will review your responses and assess psychological readiness and behavioral health considerations related to surgery.
- Step 4 — Receive Evaluation Report: Your written PDF evaluation report is typically delivered electronically within 24–48 hours after the clinical interview. Please provide the report directly to your bariatric surgery team.
